Send Patient Intake Forms to the patient before the consult

Gold Plan members have the ability to send Patient intake forms to the patient’s email or SMS/text (if your SMS/text number is enabled). To do so, go to any Encounter Details → Click on Actions → Send Pt Intake Forms.

Select the way through which you want to send the form to the patient.

This will automatically send a secure link to the patient. When the patients will click on the link, they will be asked to verify their DoB, after which they will be able to fill the information. The patients also have the option to attach insurance ID card or driver’s license image that you request in a HIPAA compliant way. If the patients are using their phone to fill the patient intake form, then they can also take the image using the camera of their phone.

 

Once the information is submitted through patient intake form, it will appear in Session Details. The documents that patient attaches will appear in Documents tab.

 

The provider’s have the ability to copy the data from session details or patient intake form to their HPI/PP notes (Subjective) by clicking on Copy from Encounter Details button

 

 

DocVilla also has the ability to send Patient Intake forms via email directly to the patients whenever a new consult is created. To do so, Click on Setup → Enc Setup → Misc → Select “Send Intake form automatically to the patient when a new appointment is created” and click Save. Whenever a new consult is created, the DocVilla system will automatically email Pt intake form to the Patient via email.
