Sending forms through the EMR is a streamlined way to collect and manage patient information. Here’s a detailed guide:
Steps to Send Consent Forms from EMR to Patients
Step 1: Log In to Your EHR Dashboard
Open the EHR system and log in with your credentials.
Step 2: Access the Patient Profile
Navigate to the Patient section.
Select the specific patient’s profile to view their information.
Step 3: Go to Forms Section
Navigate to the Forms tab in the patient’s profile.
Click on the Available option to view all forms.
Step 4: Select and Send the Form
Choose the desired form (e.g., consent form, medical history).
Select the Email or SMS option to send the form to the patient. You can also send the consent forms to the patient portal.
Consent forms can also be automated from the Setup screen to be sent automatically whenever a patient is added.
Step 5: Patient Verification and Submission
The patient receives the form via their selected method.
To access the form, they enter their date of birth for verification.
They complete the form, add a signature and date if required, and submit it.
Step 6: Review Completed Forms
Return to the Forms section in the patient’s profile.
Go to the Sent tab to check the form’s status.
Forms marked as Signed will display the date and time of submission.
Step 7: View and Manage Completed Forms
Click on the signed form to view its contents.
Save or use the information as needed for patient care.

Forms are customizable for different use cases (e.g., patient intake, consent).
This feature ensures efficient, paperless communication and seamless integration of patient data into the EMR.
