How to Book an Appointment from the Custom Patient Portal
This step-by-step guide outlines how to book an appointment through your custom patient portal, ensuring smooth scheduling and accurate data entry.
Steps to Book an Appointment
Step 1: Log In to the Patient Portal
Access the portal and navigate to the “Appointments” section to see existing bookings or start a new one.
Step 2: Click on “Take Appointment”
Select the Take Appointment option to initiate a new appointment request.
Step 3: Select a Provider and Location
Choose the desired doctor or healthcare provider.
Pick the location for the appointment (in-person or virtual).
Step 4: Choose Service and View Details
Select the type of service (e.g., consultation, follow-up).
Review the pricing, duration, and any other relevant information.
Step 5: Pick the Date and Time
Use the Calendar to find available slots.
Choose the desired date and time based on the provider's availability.
Step 6: Enter Patient Information
Personal Details:
First Name, Middle Name, Last Name
Date of Birth, Gender, Race, Address
Contact Information:
State, Mobile Number
Health Information:
Height, Medications, Supplements
Allergies, Tobacco/Alcohol Usage
Current Health Issues, Primary Complaints, Additional Notes
Insurance:
Provide insurance details (if applicable).
Step 7: Enter Payment Information
Enter credit card details:
Name on Card, Card Number, CVV, and Expiry Date.
Note: This step is mandatory to complete the booking.
Now click on Book Appointment.
In the custom patient portal, we have ability to remove the insurance section for cash based practices.
Step 8: Appointment Confirmation
After submission, the appointment will be sent for the doctor’s approval.
Once approved, it will appear in your Appointments section.
